Key Responsibilities:
- Conduct in-home visits, health assessments, and care planning for members with acute and chronic conditions
- Collaborate with other healthcare providers to deliver high-quality, patient-centered care
- Participate in call rotation of telephonic on call coverage after hours
Requirements:
- Bachelor’s in nursing with Illinois APRN license (Master’s preferred).
- Board certification for Nurse Practitioners and DEA licensure.
- Valid driver’s license and proof of insurance.
