Summary
As a Registered Nurse Case Manager at White Orchid Hospice, you will play a vital role in coordinating patient care and ensuring that our clients receive personalized and effective treatment. This position is essential for maintaining the quality of life for our patients while supporting their families during challenging times.
Responsibilities
- Develop and implement individualized care plans for hospice patients.
- Coordinate with interdisciplinary teams to ensure comprehensive patient care.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and support to patients and their families regarding end-of-life options.
- Conduct regular assessments of patient needs and preferences.
- Manage medication administration and symptom control effectively.
- Document all patient interactions and updates in compliance with healthcare regulations.
Requirements
- Current Registered Nurse (RN) license in the state of Texas.
- Hospice Experience for 1 year.
- Experience in hospice or palliative care preferred.
- Strong knowledge of geriatrics and primary care practices.
- Excellent communication skills for effective interaction with patients and families.
- Ability to work collaboratively within a healthcare team environment.
- Strong organizational skills with attention to detail.

Job Type: Contract
Pay: $47,570.85 - $57,289.63 per year
