The Program Manager plays a critical role in the day-to-day management and oversight of specific programs or services within their identified Division. Reporting to the Director of Programs or Executive Director, this position is responsible for coordinating program activities, supervising host families and staff (where applicable), and ensuring the delivery of high-quality services to program participants. This role includes a caseload of program participants. This position is responsible for:
Staff Management:
- Supervise and engage host families and staff (where applicable), providing in person direction, guidance, and support.
- Conduct regular visits to host families homes to discuss program goals, updates, and challenges.
- Actively participate in program and orientation for host families.
- Provide training and professional development opportunities for host families and staff members(where applicable).
- Delegate tasks and responsibilities to team members, ensuring workload balance.
- Address staffing issues, conflicts, and concerns in a timely and effective manner.
- Foster a positive and inclusive work environment that promotes teamwork, engagement and collaboration.
Client Care and Safety:
- Oversee participant intake and assessment processes to ensure a comprehensive understanding of individual needs.
- Develop and implement individualized service plans for program participants.
- Monitor participant progress towards goals and adjust service plans as needed.
- Provide crisis intervention and support to participants experiencing emergencies or crises.
- Ensure participant’s safety and well-being through regular check-ins and follow-ups.
- Coordinates healthcare and medication oversight
Program Operations:
- Overall coordination of individual care and services, including programattic and medical supports.
- Ensure compliance with programmatic requirements, regulations, and standards.
- Collect, analyze, and report program data and outcomes to stakeholders.
- Coordinate program logistics, including scheduling(where applicable).
- Collaborate with community partners and stakeholders to enhance program impact.
- Represent the program and organization in meetings, events, and community forums.
- Demonstrate initiative & personal responsibility for assignments; comply with state, federal and agency expectations, and regulations.
- Responsible for personnel management to include but not limited to: Leave Management, Workers Compensation, Unemployment, Performance Management, hiring, remediation and, termination.
- Management of Professional services.
- Purchasing of required supplies and reconciliation of receipts.
- On-call responsibilities for identified regions.
- Approve host family payments and DSP timecards (where applicable).
- Incident Management in partnership with PQI and QCO.
- Where applicable, an understanding of labor to include but not limited to: seniority and overtime rules and contract interpretation.
- Oversight of team compliance with required training.
- Oversight of program operations to ensure adherence to monthly budgetary requirements.
- Meets with Supports Coordinators (SC)/Case Manager to transfer ISP data
- Collaborate with Behavior Specialist regarding Behavior Support Plan; submits to Peer Review; works with management regarding response and plan of action; monitors adherence to plan
- Participates in the development of Supports Intensity Scale (SIS)
- Analyzes daily, weekly, monthly, progress notes and data collection tools to verify adherence to ISP; determines any changes needed or inconsistencies in services provided; and initiates changes as appropriate
- Responsible for service plan development and implementation
- Completes and coordinates annual consents
- Works with business authorization and processes department on individual admissions, transfers, and discharges
- Compiles individual specific data such as medical history, units of service, rates, staffing needs, etc. for all initial/annual pre-ISP assessments, including HRST
- Work a flexible schedule, including some evenings and weekends, to assure support and compliance as needed.
- HRST (PA requirement) development and coordination
- Completes and coordinates annual consents
- Supports the coordination of Medicaid renewals and redeterminations
- Coordinates and facilitates Human Rights Meetings (NJ) or the equivalent in other states
- Completes the coordination the annual assessments
- Conducts audits to ensure compliance with licensing regulations
- Collaborates with Nursing to address Consumer healthcare issues, prescribed medications, discharge instructions, Consumer-specific adaptive equipment needs, and delivery of Individual -specific training
- Provides follow-up regarding medical appointments, cancelled appointments, transportation, and medical refills, etc.
- Complies with Incident management policy and procedures to include any necessary follow-up

QualificationsThe ideal candidate will possess the following qualifications:
- A master’s degree or above from an accredited college or university and 1 year of work experience working directly with persons with an intellectual disability or autism.
- A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with persons with an intellectual disability or autism.
- An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with an intellectual disability or autism.
- A high school diploma or general education development certificate and 6 years of work experience working directly with persons with an intellectual disability or autism.
