Overview
We are seeking a dedicated and compassionate Psychiatric-Mental Health Nurse Practitioner to join our team as soon as possible. This role involves providing comprehensive mental health care to patients in a long term care and rehabilitation setting. The ideal candidate will be skilled in assessing, diagnosing, and treating psychiatric conditions, have an understanding of handling complex patients and geriatrics, while collaborating with a multidisciplinary team to ensure optimal patient outcomes.
Duties
- Conduct thorough psychiatric evaluations and assessments for patients.
- Develop and implement individualized treatment plans based on patient needs.
- Provide medication management, including prescribing and monitoring psychotropic medications.
- Offer basic psychotherapy and support to promote mental wellness.
- Collaborate with other healthcare professionals in internal medicine, infectious disease care, and memory care to provide holistic patient care.
- Maintain accurate notetaking, documentation and coding of patient records using ICD-10 standards.
- Educate patients and families about mental health conditions and treatment options.
- Incorporate advanced diagnostics such as PGx and genetics when indicated (training provided).
- Participate in continuous professional development and training to stay current with best practices in psychiatric care.
Qualifications
- Current certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) in the state of New Mexico
- Experience with nursing home setting and/or geriatrics is preferable
- Strong knowledge of psychiatric assessment tools and treatment modalities.
- Excellent communication skills, both verbal and written, with the ability to build rapport with patients.
- Ability to work collaboratively within a multidisciplinary team environment..
