Overview
The Registered Nurse Case Manager plays a critical role in coordinating patient care across various healthcare settings. This position requires a compassionate and skilled nursing professional who can effectively manage patient assessments, discharge planning, and care transitions. Duties
- Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
- Coordinate care across multiple disciplines, including primary care, specialty services, and post-acute care.
- Facilitate discharge planning by collaborating with healthcare teams to ensure smooth transitions for patients.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide education and support to patients and families regarding treatment options and health management.
- Utilize critical thinking skills to evaluate diagnostic information and implement appropriate interventions.
- Engage in utilization management activities to optimize resource use while maintaining high-quality patient service.
Skills
- Critical care experience, including ICU and emergency room settings.
- Familiarity with Level I and Level II trauma centers, as well as pediatric intensive care (PICU).
- Proficient in hospital medicine, infectious disease care, geriatrics, behavioral health, medical-surgical nursing, and outpatient clinic experience.
- Strong knowledge of medical terminology, physiology, discharge planning processes, and case management principles.
- Experience in managed care environments with a focus on patient assessment and service delivery.
- Ability to perform vital signs monitoring and diagnostic evaluations effectively.
- Excellent communication skills for effective collaboration with healthcare teams and patient education.
- Understanding of post-acute care dynamics and the ability to navigate complex healthcare systems. This position is essential for ensuring that patients receive comprehensive support throughout their healthcare journey.
