We have a fantastic opportunity for a RN Case Manager to join our team.
Responsibilities:
- Management of patient population to optimize utilization
- Discharge planning
- Coordinate with physicians, nurses, physical therapy and ancillary staff to facilitate all departmental processes.
- Communicate with Home Health Agencies and Managed Care Plans, provide clinical information for admission and follow-up patient reviews.
- Ensure payer documentation and information requirements are met.
Education and Requirements:
- New York state License for Registered Professional Nurse and current registration or valid permit to practice in new York state as an R.N.
- BSN degree required
- Minimum of one of experience required in an acute care setting
- Knowledge of Medicare Regulations
- Excellent interpersonal and communication skills
- Experience with electronic health records
