RN Case Manager

Team Select Home Care is currently seeking a dedicated and compassionate RN Case Manager to join our growing family. As an integral part of our team, the RN Case Manager will play a crucial role in providing exceptional care to our patients. This position involves a dynamic blend of office and field responsibilities, offering a unique opportunity to engage with both our healthcare professionals and patients.

Responsibilities:

Patient Care:

• Conduct initial evaluation visits and reassess patients' nursing needs regularly.
• Coordinate and collaborate with healthcare professionals to develop and implement individualized care plans.
• Monitor patient progress and adjust care plans as needed.
• Provide hands-on patient care when necessary.

Documentation:

• Prepare accurate and timely clinical and progress notes for each patient visit.
• Summarize care conferences promptly following agency policies and procedures.
• Maintain comprehensive and up-to-date patient records.

Communication:

• Collaborate with the Director of Nursing to ensure effective communication and coordination of care.
• Communicate with patients, families, and healthcare team members to provide updates and address concerns.

Field and Office Time Management:

• Effectively balance time spent in the office and the field.
• Attend regular team meetings and training sessions.
• Ensure compliance with agency policies and standards.

Qualifications:

• Active and unrestricted RN license in the state of North Carolina.
• Knowledge of Pediatric care.
• Comfortable with using a computer.
• Strong communication and interpersonal skills.
• Knowledge of home healthcare regulations and standards.
• Willingness to be on call as needed.
