Job Type: Full-Time
Responsibilities
- Conduct comprehensive patient assessments to develop and implement individualized care plans in hospice settings.
- Collaborate with interdisciplinary teams to ensure optimal patient service and care continuity.
- Manage patient caseloads, ensuring compliance with HIPAA regulations and medical terminology standards.
- Facilitate discharge planning and coordinate post-acute care services for patients and their families.
- Provide education and support to patients and caregivers regarding hospice care options and resources.
