All Star Home Health Care is a small home care agency looking for an RN Case Manager.
Duties to include:
- Assessments
- In home and phone visits with clients, and documentation
- Train and supervise support staff. Provide quarterly evaluations
- Develop and update client "Support Plans" based off of county assessments
- Assist with implementation of EVV-electronic timesheets
The hours are part time and very flexible, set your own hours! This is a supervisory position.
QUALIFICATIONS:
- Current license as a registered nurse in the State of Minnesota
- Must have a reliable vehicle
