Job Description:
Description: The RN Certified Diabetes Educator provides clinical nursing assessment, care and education to patients with diabetes and their families. Including developing, coordinating and evaluating educational material and resources for the diabetes population. The RN Certified Diabetes Educator provides individual and group education sessions for the patients and works cooperatively with the Network providers and staff to accomplish the goals and objectives of patient care according to established policies and procedures..
Typical Duties
Works with all appropriate members of the health care team to ensure coordination of care for patients with diabetes through the development and use of patient care and practice standards.
Ensures desired patient outcomes achieved and variances from established parameters are evaluated and addressed as necessary.
Focuses on optimal management of the diabetic patient with education and resources that target individualized educational needs of the patient and family to improve patient satisfaction with the diabetes educational process.
Provides direct care and consultation around discharge planning issues and patient/family teaching.
Assists in the process of meeting planning, diabetes presentations to various groups and committees, preparation of materials and maintenance of records and data.
- Performs all other related job duties as assigned.
Qualifications:
- Bachelor’s Degree in Nursing, from an accredited college or university.
- 4 plus years of healthcare experience including 2 years of clinical work experience.
- Bilingual to include Spanish and English fluency
- All applicants must possess a valid Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES) credential in accordance with the minimum qualifications set by the Certification Board for Diabetes Care and Education (CBDCE), formerly known as the National Certification Board for Diabetes Educators (NCBDE) within 12 months of hire.
- Current RN licensure by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state.
- Current JPS recognized Basic Life Support (BLS) Certification.
- Advanced Cardiovascular Life Support (ACLS) Certification, if applicable.
