We are currently looking for an experienced Home Health RN Case Manager for our Home Health Shift Care Department covering the LA area.
This position requires the RN to schedule visits, to Open Cases (In the Field) , Recertify, and Discharge patients once goals have been met. Medical & Medicare experience needed.
Experience: 2-3 years in Home Health , Knowledge with EMR, DSL (Data Soft Logic) Adult/Pediatric Home Health and/or Hospice is highly desired.
The RN Case Manager will be responsible for Initial Assessments, Initiating the plan of care and making revisions as the patient's needs change, Counseling patient and families in meeting nursing needs, Communicating with the physician regarding patient's needs and reporting any changes in the patient's condition, Obtaining and receiving physician's orders as required, Counseling field staff when the need arises.
Qualifications:
- Graduate of an accredited school of nursing. Previous home health/OASIS experience is a plus.
- Current licensure in California, CPR certification and valid drivers license. Current physical and TB test required. Must be able to pass a background check.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
