Overview
We are seeking a dedicated and compassionate RN or Licensed Practical Nurse (LPN) to join our Long Term Care team. The ideal candidate will provide high-quality nursing care to patients in various settings, ensuring their comfort and well-being.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor patient vital signs and report any changes to the healthcare team.
- Assist with daily living activities, including bathing, dressing, and feeding patients.
- Provide catheter care and support for patients requiring specialized medical attention.
- Participate in discharge planning to ensure a smooth transition for patients leaving care facilities.
- Collaborate with healthcare professionals to develop and implement individualized patient care plans.
- Educate patients and their families about health management, home community care, and post-discharge instructions.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
Qualifications
- Valid RN or LPN license.
- Knowledge of physiology and basic medical principles relevant to nursing care.
- Strong communication skills and ability to work collaboratively within a healthcare team.
