Join The Community Integration Project’s team of dedicated service providers who seek to promote the accessibility of services that improve quality of life and support self-determination for people with mental and developmental disorders. This job provides recovery management services to individuals with severe mental illness (SMI) strictly adhering to the Home & Community Based model as outlined in the HCBS Provider Manual and other HHSC guidelines. RM will provide recovery management at an intensive level, three contacts per week, for the first three months of an individual’s participation in the program and complete the Individual Recovery Plan according to the individual’s current needs and desire, including assisting individual’s in selecting a provider agency outside of the Local Mental Health Authority, if applicable. RM will monitor the health, welfare and safety of individuals enrolled in HCBS through regular contacts and respond to emergency/crisis situations as appropriate. Advocacy in the community for all individuals assigned to HCBS services is critical to this role, as well as programs and activities which promote independence and community integration. RM will coordinate with the referring entity (State Hospital Staff, LMHA) to assess client needs and continuity of care. RM will maintain and ensure that staff development and training meet the required standard set forth by CIP and HHSC. Operates under trauma-informed and recovery-oriented models of care which align with Community Behavioral Health standards.
· Education and/or Experience: • master’s degree in human services or related field with two (2) years’ Human Services experience Certificates, Licensure, Registrations:
· • Current driver’s license and vehicle for use while at work.
This is a 1099 contractor position. Direct Billing is reimbursed at $50 an hour.
