Summary of Duties:
Using own automobile, assists Hospice patients/families to meet their psychosocial needs through assessment and appropriate intervention as needed. Acts in a consultant role to multidisciplinary staff members in Hospice. Furthers the knowledge and understanding of the social work role of Hospice through appropriate educational outreach.
Essential Functions of the Job
- Evaluates the psychosocial needs of referred patients/families, taking into consideration: developmental history, current family system, need for services or community referrals.
- Provides assistance with bereavement coping.
- Provides social casework and counseling as needed.
- Participates in the development of the Patient/Family Interdisciplinary Plan of Care, with particular attention to emotional, social, and family needs.
- Provides the social work perspective at Interdisciplinary Group conferences. In cooperation with other disciplines, develops and maintains a plan of care which meets the needs of the patient/family.
- Assists with Hospice Bereavement Services with a complete follow-up.
- Ensures that all patient information is held in strict confidence.
- Performs related administrative functions and other related duties as assigned.
Educational Requirements: Master's in Social Work. Current licensure as a Social Worker in the State of Hawaii.
Experience: One year’s previous experience. Hospice - Death & Dying experience, preferred.
Skills Required: Current Hawaii driver's license with clean Driver's Abstract. Excellent oral and written communication skills. Ability to both organize and prioritize work. Strong interpersonal relationship skills. Documented computer skills.
