Job Summary
Role and Responsibilities
Assess patient health problems and needs, develop and implement nursing care plans. Advise patients on health maintenance and disease management. Medical problem solving with physician collaboration.
- Consult and coordinate with with health care team members to assess, plan implement and evaluate patient care plans.
- Order, interpret and evaluate diagnostic tests to identify and assess patient’s condition.
- Maintain accurate, detailed reports and medical records.
- Greet and bring patients into the exam room, interview for chief complaints, reconcile medications, update medical, surgical, social and family history, nursing assessment including review of systems and vital signs.
- Expedite cardiac clearance, home care orders, cardiac rehab and visiting nurse services.
- Supervise and maintain patient flow during office hours.
- Patient teaching.
- Maintain sample medication logs.
- Telephone triage and patient assessment.
- Review and expedite insurance coverage compliance requirements for patient care.
