Job Summary
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care across various departments, ensuring that patients receive the necessary services and support throughout their treatment journey. This role is vital in promoting optimal health outcomes for patients, particularly within a Level II Trauma Center environment. The Case Manager will work closely with medical professionals, patients, and families to facilitate effective communication and care planning.
Duties
- Assess patient needs and develop individualized care plans based on medical history and current health status.
- Coordinate with healthcare providers in acute care settings, including ICU and PICU, to ensure comprehensive patient management.
- Maintain accurate medical records and documentation, adhering to medical terminology standards.
- Provide education and support to patients and their families regarding treatment options and resources available.
- Collaborate with multidisciplinary teams to facilitate smooth transitions of care and discharge planning.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Advocate for patients' rights and ensure they receive appropriate services in a timely manner.
Skills
- Strong understanding of patient service principles within a healthcare setting.
- Familiarity with case management processes, particularly in pediatrics or trauma care.
