Position: Transition Center 1-1 Paraprofessional - Life Skills, Pre-Employment/Job Coaching, and Community Integration
Location: Transition Center/In the community and potentially in the home
Schedule: 32.5 hours per week

We are seeking a dedicated and compassionate individual to work one-on-one with a young adult with disabilities, focusing on developing essential life skills, pre-employment readiness, independent living skills, and community navigation. This role is highly individualized and requires a proactive, patient, and creative person who is passionate about fostering independence and inclusion. The successful candidate will work collaboratively with the individual, their family, and relevant service providers to implement personalized goals and support their growth. Key Responsibilities:
- Life Skills Development:
- Assist with daily living activities such as personal hygiene, meal preparation, household chores, and organization.
- Support the development of budgeting, financial literacy, and basic money management skills.
- Promote healthy habits, including nutrition and basic self-care.
- Encourage problem-solving and decision-making skills in everyday situations.
- Pre-Employment Skills Training:
- Support the development of foundational work readiness skills, including punctuality, attendance, following instructions, and task completion.
- Assist with resume building, job searching strategies, and application processes.
- Practice interview skills and appropriate workplace communication.
- Help identify and explore potential vocational interests and opportunities.
- Community Navigation and Integration:
- Facilitate safe and independent travel within the community (e.g., public transportation, walking routes).
- Support participation in recreational activities, social events, and community programs.
- Assist with accessing community resources such as libraries, grocery stores, and healthcare facilities.
- Promote appropriate social interactions and communication in various community settings.
- Help the individual advocate for their needs and preferences in public spaces.
- Documentation and Collaboration:
- Maintain accurate and timely documentation of progress, challenges, and interventions.
- Communicate regularly and effectively with the individual, family members, and other support professionals (e.g., case managers, therapists).
- Participate in team meetings and contribute to the development and revision of individualized plans.
- Adhere to all confidentiality and ethical guidelines.
Qualifications:
- High school diploma or GED required; Associate's or Bachelor's degree in a related field (e.g., Special Education, Human Services, Psychology) preferred.
- Prior experience working with individuals with disabilities, particularly young adults, strongly preferred.
- Demonstrated understanding of best practices in supporting individuals with disabilities in developing life skills, pre-employment skills, and community integration.
- Ability to implement individualized plans and adapt strategies based on the individual's needs and progress.
- Excellent communication, interpersonal, and active listening skills.
- Patience, empathy, and a positive, encouraging demeanor.
- Ability to maintain professional boundaries and confidentiality.
- Reliable transportation and a valid driver's license with a clean driving record [if community travel is required] and ability to get van certified by the school district. This postion will require transportation of the student.
- Successful completion of a background check.
Physical Requirements:
- Ability to perform activities such as walking, standing, bending, and occasionally lifting up to [specify weight, e.g., 25] pounds.
- Ability to work in various community settings, which may include indoor and outdoor environments.
Compensation: $21.70 per hour, depending on qualifications and experience.
