The Transition Coordinator plays a vital role in facilitating the smooth transition of individuals, particularly those with disabilities, into new programs or services. Responsibilities
- Coordinate the transition process for individuals moving into new programs or services.

- Supervise staff involved in the transition process, providing guidance and support as needed.
- Manage program development efforts to ensure alignment with best practices and regulatory requirements.
- Oversee budgeting processes related to transition programs, ensuring financial accountability.
- Maintain strong relationships with stakeholders, including families, service providers, and community organizations.
- Conduct assessments to identify the needs of individuals transitioning into new services.
- Facilitate training sessions for staff on best practices in supporting individuals with developmental disabilities.
- Ensure compliance with all relevant policies and regulations throughout the transition process.
Requirements
- Bachelor’s degree in Social Work, Psychology, or a related field
- Proven experience in supervising teams and managing programs within the behavioral health or social services sectors.
- Strong skills in program development and budgeting.
- Excellent relationship management abilities to effectively engage with individuals, families, and community partners.
- Knowledge of best practices for working with people with developmental disabilities.
- Exceptional organizational skills and attention to detail.
- Ability to manage multiple priorities in a fast-paced environment.
- Strong communication skills, both verbal and written.
