We're hiring immediately for dedicated providers who can start within 2 weeks.
Position Highlights:
- $65 per Telehealth Visit
- $80 per Standard Home Visit
- Up to $100 per Complex Wound Care Visit (wound care experience strongly preferred)
- Flexible schedule: Work 2–3 days/week between 9 AM – 5 PM
- Part-time, as-needed position (No benefits)

What You’ll Do:
- Conduct medical visits in patient homes
- Manage chronic conditions and patient assessments
- Perform wound care and complex case management (if applicable)
- Document accurately in EMR
- Deliver compassionate, high-quality care
Requirements:
- Licensed Nurse Practitioner (Illinois) – Required
- Experience in chronic disease management
- Comfortable working independently in patient homes
- Reliable, organized, and mission-driven
- Wound care experience is a plus and increases compensation
