We are currently seeking a skilled and compassionate Family Nurse Practitioner (FNP) with expertise in wound care to join our growing team. Our clinic provides both in-office and home-based wound care services, ensuring patients receive exceptional, evidence-based treatment that promotes healing and minimizes complications.
This role focuses on delivering advanced wound care management, including the assessment, diagnosis, and treatment of acute and chronic wounds. The ideal candidate will have a background in wound care or home health, a patient-centered approach, and the ability to work both independently and collaboratively with our care team.
Position Overview
As a Family Nurse Practitioner (FNP) specializing in Wound Care at UpBeat Health LV, you will play a key role in managing complex wounds across diverse patient populations in both clinic and home health settings. You will assess and treat a variety of wound types including pressure injuries, diabetic ulcers, venous and arterial wounds, and post-surgical wounds, with an emphasis on evidence-based interventions and patient education.
This is a dual-setting role, providing flexibility and diversity in your day-to-day responsibilities. You’ll manage patients in our clinic and through home visits, supporting those with mobility challenges, chronic illnesses, or limited access to traditional healthcare. Your focus will be on healing outcomes, infection prevention, and improving patients’ quality of life.
Salary:
- Annual Salary Range: $140,000 - $160,000

Key Responsibilities:
- Perform thorough wound assessments and develop individualized treatment plans.
- Administer and monitor wound care treatments including debridement, dressing changes, and application of advanced therapies.
- Provide wound prevention education and collaborate with patients and caregivers on care plans.
- Prescribe appropriate medications and treatment regimens in accordance with clinical guidelines.
- Order and interpret relevant labs, diagnostics, and imaging to support clinical decisions.
- Collaborate with an interdisciplinary team, including physicians, nurses, and home health providers.
- Maintain detailed and timely documentation using EHR systems.
- Monitor wound progression and modify care plans as needed to optimize healing.
- Participate in clinical quality improvement initiatives related to wound care outcomes.
Qualifications:
- Education: Master’s degree in Nursing with a specialization as a Family Nurse Practitioner (FNP).
- Licensure: Active, unrestricted California State FNP license.
- Experience: Minimum of 2 years of experience as an NP
- Certification: Board Certification as a Family Nurse Practitioner (FNP-BC) through an accredited organization (e.g., AANP or ANCC).
Preferred Qualifications:
- Experience in home health services or a similar setting.
- Ability to work with an interdisciplinary team.
- Bilingual is a plus.
- Strong clinical assessment, diagnostic, and treatment skills.
- Ability to work independently, make clinical decisions, and manage patient care in a home health environment.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.
