The primary responsibility of the Youth and Family Wraparound Team Lead is to serve as a mentor for Wraparound Facilitators, providing technical assistance and hands-on support to Facilitators to implement high fidelity Wraparound principles and key elements. In addition to Team Lead responsibilities, this position also performs Wraparound Facilitator duties as outlined in the Wraparound Facilitator job description.


Duties & Responsibilities:


  • Adhere to all current federal and state laws, as well as CFGC Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA)
  • Comply with initial and annual training requirements
  • Attend staff/agency meetings as required
  • Maintain cooperative working relationships with all agency staff and community partners
  • Perform duties without violating any ethical codes, state or federal codes or requirements
  • Demonstrate and promote a positive and strengths-based attitude and organizational culture
  • Provide mentoring and serve as a role model for Wraparound Facilitators, giving special attention to new hires
  • Assist in training new staff by shadowing, mentoring, and other on-the-job training until Facilitator is ready to work independently
  • Engage in field observations with each assigned (established) Wraparound Facilitator at minimum two times per month; debrief with Facilitator following observation
  • Fulfill all Wraparound Facilitator job duties as outlined in the Wraparound Facilitator job description while maintaining a caseload of 5-6 children/families
  • Provide a minimum of 8 hours of services per month per child/family to include weekly face-to-face contacts with the child and family in the community.
  • Complete all documentation per company guidelines.
  • Be part of a rotation to answer YES Inquiry calls per program requirements
  • Maintain an empathic, yet professional, relationship with children and families and strive to keep families engaged in all services
  • Maintain accurate and concise documentation and complete assessments, case notes, database documentation, wraparound and transition plans in a timely manner per program standards
  • Assume additional family cases temporarily as needed to handle growing caseloads and cover other employee absences
  • Receive consistent coaching support from the Wraparound Supervisor and complete tasks assigned in supervision meetings
  • Assist with community networking to promote and further develop the organization and program
  • Act as an agency representative at interagency meetings of providers as assigned
  • Assist in interviewing potential employees and make employment recommendations
  • Provide staff training, as needed
  • Complete other duties as assigned by the YES Waiver Program Director and/or Supervisor

  • Possesses computer skills with proficiency in data entry and keyboarding skills
  • Able to work in a high volume and fast paced
  • Ability to work a flexible schedule.
  • Self-motivated, detail-oriented individual with a team player
  • Ability to lead, train and mentor Wraparound Facilitators
  • Perform job responsibilities without violating ethical, state or federal codes

Education Requirements:

  • Must possess a minimum of a Bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas Administrative Code for QMHP-CS qualifications.
  • General understanding of child mental health disorders, treatment of disorders, and documentation of treatment provided. Knowledge of confidentiality and ethical guidelines for practice.
  • Knowledge and understanding of the Wraparound (NWIC) model.

Other Skills & Qualifications:

  • Should possess basic therapeutic skills, oral and written communication skills, and organizational skills.
  • Must be goal oriented, motivated, capable of working independently and using good judgment.
  • Must possess a strong understanding and ability to work with diverse populations including various cultures, racial groups, and socio-economic levels.
  • Must have a working knowledge of Microsoft Outlook, Excel, Word, and Medisoft billing and scheduling system or similar product.
  • Ability to relate to consumers, employees, and the general public in a professional manner.
  • Ability to follow verbal and written instructions. Must be able to prioritize.
  • Ability to adhere to articulated policy and procedures.
  • Ability to provide home visits in the community as required in own vehicle.
