Foundations Health & Wholeness is adding a Youth System (RAYS) Navigator. The mission is to keep youth and teens (school-age through age 21) healthy and safe primarily through prevention outreach activities in youth-populated spaces across these communities. Additionally, we engage with youth and teens through street outreach, providing both preventative and intervention/crisis services as needed.
The Youth System Navigator is a remote position and will be focused on Forest, Langlade, Oneida and Vilas counties. The position is responsible for assessing unaccompanied youths’ needs, addressing immediate client concerns, and providing ongoing supportive services and connections to resources to achieve outcomes toward safety and stability. The position ensures adherence to the Youth Homelessness Demonstration Program (YHDP) legislation through the U.S. Department of Housing and Urban Development (HUD) funding requirements. The position uses a trauma-informed care approach and best practice techniques to develop programming for youth during community outreach activities and case management. The System Navigator provides referrals and introductions to community resources to assist youth and their families.
Duties and Responsibilities:
- Assess client needs, develop individualized care plans, coordinate service referrals and services, monitor process and advocate for clients
- Support the local Youth Advisory Board (YAB) by facilitating meetings, reviewing and approving reimbursements, recruiting youth to participate in the local YAB, identifying additional leadership opportunities, and being the point of contact between the local YABs and the CoC Staff
- Document and track data for files and grant retention purposes
- Collaborate with community agencies, resources, and services within the local coalition and participate in community committees relevant to the grants that fund the RAYS program, including communicating barriers youth are encountering with the Balance of State (BOS) at the local level
- Oversee and coordinate the daily programming of outreach services activities, presentations, groups, client service needs (outreach/shelter/small "c" counseling/case management/and referrals), and referrals to gateway services (shelter, food, transportation, hygiene, medical and referral assistance)
- Provide education/outreach and assist with the creation/distribution of marketing materials/social media as is required by the program grants so youth in the community know the brand
- Facilitate training and discussions with stakeholders and other community partners around the unique needs and barriers experienced by youth in the community
- Assist in coverage of the helpline as required by the grants, providing 24/7 availability to clients
- Weekly or monthly travel to meeting, trainings, etc., is required
Experience and Education:
- Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field preferred
- Possess and maintain valid driver's license
- Experience working with and providing support services to vulnerable youth
- Experience in social work, psychology, or related field
- Knowledge and training in the areas of positive youth development, crisis intervention, trauma-informed care, strength-based interventions, and conflict resolution
- Ability to work well with many different levels/organizations to achieve a common goal
- Excellent advocacy and communication skills
- Experience in case management and/or assessment
- Experience with data collection and/or data entry
- Experience measuring and meeting grant outcomes
