To be a successful Adoption Case Manager you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide services that result in timely and safe achievement of company and state outcomes relevant to the position.?
- Provide direct practice and case management services to include child visits and monthly supervision of the home.
- Help prepare children for adoption and make referrals to the Adoption Therapist. Update the child’s social history, child’s plan of service, attend court hearings, complete the sibling split request, and review the child’s file with the selected family.
- Assist in matching adoptive families with children eligible for adoption.?
- Support adoptive homes and ensure all required trainings, documentation, and background checks are completed.?
- Provide training and preparation for adoptive families.??
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
- Back up on-call to the on-call department.
- Provide support for the children we serve by ensuring they attend their required appointments and by providing transportation if needed.
- Maintain flexibility during business hours to address any unexpected needs of the children we serve.

To qualify for this job, you need to have:
- Bachelor’s degree required in social services or related field.
- Adoption experience preferred.
- 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
- Maintain company-required training.
- Physical working requirements - Ability to travel, drive Company or personal vehicle including at night. Ability to do occasional lifting of up to 50 lbs. Able to walk stairs. Ability to work flexible schedule, including some evenings and weekends. Employees are subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
