Job Overview
We are seeking a dedicated and organized Care Coordinator to join our team. Responsibilities
- Serve as the primary point of contact for patients, addressing inquiries and providing information regarding services.
- Coordinate field staff schedules, ensuring patients are visited and services are provided as ordered
- Maintain accurate patient visits records and documentation in compliance with healthcare regulations.
- Assist with insurance verification and authorization processes to facilitate patient care.
- Assist home health teams in implementation of individualized care plans tailored to patient needs.
- Provide administrative support, including managing correspondence, filing, and data entry.
- Ensure a smooth flow of communication between patients, field staff, and administrative teams.
Qualifications
- Previous experience in home health care setting or care coordination is preferred.
- Familiarity with medical terminology is essential for effective communication within the healthcare environment.
- Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent interpersonal skills to foster positive relationships with patients and staff members.
- Proficient in office administration tasks, including scheduling, record keeping, and data management.
