Job Title: Case Manager

Reports to: Executive Director

Position Overview

A Case Manager’s primary goal is to ensure that an individual reaches their optimum level of wellness and functionality within his/her environment. Case Managers assess the needs of the individual and family and coordinate care services. This can include counseling, nutrition, rehabilitation, transportation and educational services. Case Managers also work to ensure that care is provided in the most comprehensive and cost-effective manner for the individual and their insurance providers. Case Managers then monitor the continuity of care and serve as an advocate for the individual.

Qualifications

Language Skills:

Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and analyze policies and procedures. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to lead a team and communicate effectively with employees, clients and leadership.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions.

Computer Skills:

To perform this job successfully, an individual should have basic computer skills. Basic understanding of Microsoft word suite.

● Stand, walk and sit during assigned shift hours.

● Ability to manually handle patient belongings with gloved hands.

● Be able to manually assess a patient as part of a physical exam.

● Reach, climb or balance, stoop, kneel, crouch or crawl.

● Speak and have access to the special senses within normal limits (hearing, taste, smell, etc.) as it applies to patient care.

● Capable of lifting 50 pounds.

● Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
