Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive support and guidance to clients facing various challenges, including behavioral health issues, addiction, and social welfare needs. This role requires a strong commitment to crisis intervention and management, as well as the ability to work collaboratively with clients to develop effective treatment plans and resources.
Duties
- Conduct assessments to identify client needs and develop individualized care plans.
- Provide crisis intervention and management to clients in urgent situations.
- Offer addiction counseling and support for individuals struggling with substance use disorders.
- Facilitate access to behavioral health services, including therapy and medication management.
- Supervise and mentor junior staff or interns, ensuring adherence to best practices in case management.
- Collaborate with community resources, including Section 8 housing assistance and child welfare services.
- Assist clients in navigating Medicare and other healthcare options.
- Engage in workforce development initiatives to help clients gain employment skills.
- Maintain accurate documentation of client interactions and progress in accordance with organizational policies.
Requirements
- Bachelor’s degree in Social Work, Psychology, or a related field.
- Proven experience in crisis intervention, crisis management, or related fields.
- Knowledge of addiction counseling techniques and behavioral health practices.
- Familiarity with social work principles and child welfare systems.
- Strong skills in motivational interviewing and client engagement strategies.
- Supervising experience is a plus, particularly in mentoring new staff or interns.
- Experience with childcare services or programs is beneficial.
- Ability to work collaboratively within a multidisciplinary team environment.

Job Type: Part-time
Pay: $23.78 - $28.63 per hour
Expected hours: 20 – 30 per week
