Job Description

The CRU Case Manager provides direct support to adults experiencing serious and persistent mental illness, often in the context of acute psychiatric crises requiring short-term residential care. In addition to working within the Crisis Recovery Unit, this position also includes oversight of a transitional housing program that supports client stability following discharge.

The CRU Case Manager applies a generalist intervention model to deliver services and works collaboratively with clients and multidisciplinary treatment team to:

  • Engage clients in their individualized treatment plan.
  • Conduct assessments to identify unmet needs
  • Plan and implement targeted interventions
  • Coordinate case with service providers
  • Assist in monitoring clients' progress
  • Provide consistent follow-up to promote stability and wellness

This role requires strong communication, critical thinking, and organizational skills, as well as a trauma-informed, client-centered approach to care.

Essential Job Functions

The CRU Case Manager must be able to complete the following essential job functions:

  • Provide direct case management services to adult clients at the CRU
  • Conduct assessments to identify areas of deficiency and need
  • Engage clients in developing and implementing individualized treatment plans
  • Coordinate with multidisciplinary treatment team
  • Routinely engage with the client in the community by linking with community partners
  • Provide psychoeducation through leading individual skills sessions and group skills sessions
  • Assist in enrolling clients in government aid programs
  • Engage in crisis management
  • Attend client staffing and family team meetings as required
  • Maintain certifications, attend regular training, and complete all CEU requirements
  • Meeting agency productivity standards (68 hours per month)
  • Maintain clinical documentation as required by agency standards
  • Other duties and responsibilities and needed and assigned.

Position-specific Job Functions

In addition to the essential functions above, the CRU Case Manager also:

  • Serves as the primary point of contact for the agency's Transitional Housing Program
  • Coordinates client admissions, discharges, and length of stay planning for transitional housing
  • Conducts regular check-ins and home visits to monitor client progress and housing stability
  • Maintains compliance with housing program guidelines and documentation requirements
  • Other duties and responsibilities and needed and assigned.

Introductory Period

All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.

Employment Classification & Schedule

This position is classified as full-time, non-exempt, with a 40-hour per week schedule.

Work schedules at DBH are established to meet the needs of our clients and the agency. Program Supervisors, with the approval of Program Directors, determine the specific schedules for their assigned departments and have the authority to ask employees to work a different shift and/or in a different location on a short-term or permanent basis to meet the departments' needs.

The proposed schedule for this position is Monday through Friday from 8:00 am to 4:00 pm. This position does require flexibility to meet client and program needs, including attending commitment hearings, staff meeting, and other tasks related to completing essential job functions.

Location & Work Environment

The office for the CRU is located on the DBH Layton Campus is located at 2250 North Robins Dr., Layton, UT 84041. Work locations include but are not limited to clients' homes, clients family member homes, hospital settings, courthouses, jails, DBH offices, and other locations to help the clients meet their needs.

While the CRU Case Manager will have dedicated office space within the agency, case management activities will be performed across multiple settings. This includes time spent in the CRU facility, client homes, and community locations related to transitional housing. Significant fieldwork is required, including frequent travel throughout the community to conduct outreach, attend meetings, and provide direct client services. The position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.

Compensation

The hourly rate for this position is outlined below:

BSW Degree + SSW License

$25.50 per hour base wage + 1% per year of licensure

Non-BSW Degree

$21.50 per hour base wage + 1% per year of direct social work experience, up to 10 years.

No Degree + TCM Certification

$19.00 per hour + 1% per year of direct social work experience, up to 10 years.

Required Qualifications

Experience

While no previous experience is required, previous experience working in the social work field and/or working with people who have serious mental illness is strongly preferred.

Education

  • Bachelor's degree in social work or a related field is required.

Licensure/Certification

  • Strong preference for active licensure as a Social Service Worker (SSW) in good standing in the State of Utah. BHCM certification is not required for licensed individuals, OR
  • The ability to obtain Behavioral Health Case Management (BHCM) certification within 90 days of employment is required, if unlicensed.

The CRU Case Manager must complete and maintain certification in First Aid and CPR, Welle behavioral training, crisis intervention training, an complete all required training assigned to the CRU Case Manager by defined deadlines.

Professional Experience

The ideal candidate will have a working knowledge of mental health and substance use disorder services, community resources, experience working with clients that have severe mental illness, is hardworking, and can work independently as well as in a team environment. Experience in mental health case management or related behavioral health services preferred (1-2 years).

Physical Demands

Tasks require a variety of physical activities, not generally involving muscular strain, but do require frequent walking, standing, stooping, sitting, reaching, talking, hearing, and seeing. Common eye, hand, and finger dexterity are required for many tasks. Mental application utilizes memory for details, verbal instructions, emotional stability, and discriminating thinking. Periodic interaction with youth or adults in crisis requiring participation in de-escalation processes may occur causing stress or threat of physical injury.

Must be able to lift up to 50 pounds and perform a range of physical tasks including walking, standing, driving, and navigating stairs.

Position involves travel to varied community settings including hospitals, homes, courts, jails, and office spaces.

Occasional high-stress situations may occur involving individuals in emotional or behavioral crisis.

Must be comfortable working independently in the field with minimal direct supervision.
