Position: Case Manager (RN)

Position Summary: Responsible for acute care case manager management activities, insurance reviews, transfers, strong knowledge base of DRGs and appropriate admission status. Also demonstrates leadership skills, problem solving skills, and collaborative skills to work with other healthcare teams, including utilization management activities and promotion of high-quality cost effective care in a collaborative manner.

Job Duties/Responsibilities:

Evaluates admissions and hospital’s resource utilization for appropriateness and timeliness. Collaborates with all members of the healthcare team to assess or coordinate patient and education needs. Completes all insurance reviews, coordinates multidisciplinary team meetings, and participates in core measure compliance.
