SUMMARY
In consultation with and oversight of the RN Manager of Care Coordination and the Director of the Center for Care Coordination, and OCCH, the Program Specialist manages specific Center for Care Coordination projects delegated by the Manager/Director. The Program Specialist reviews times lines, project objectives, deliverables, and staffing needs to develop a plan to execute the project.
ROLE RESPONSIBILITIES
Collaborates with Manager/Director and OCCH to review scope of JPB Foundation deliverables as it relates to Goal Concordant Care in Primary Care Settings.
Provide support and guidance to JPB funded Care Coordinator in collaboration with Care Coordination Leadership (Manager/Team Lead)
Performs programmatic functions as required
Follows project management processes: define scope, review deliverables, create process, and generate timeline
Identifies challenges/barriers and develops solutions
Completes project work effectively, efficiency, and within specified time frames
Responsible for data collection and producing metrics for reporting purposes
Responsible for all reporting requirements of specifics grants
Active participation in the planning, development, and implementation of strategies that identify and generate funding and/or resource opportunities for program activities
Engages in community, state, national relationship building and sustainability
Co-facilitates/participates in regional and state collaboratives, work groups, coalitions
Advises and responds to calls for technical assistance from stakeholders/affiliates through conference calls and other modes of communication
Creates and participates in development of content for forums and conferences
Develops/prepares presentations for The Center for Care Coordination, The Office for Community Child Health, CT Children’s Medical Center for discussions and dissemination of Goal Concordant Care deliverables.
Develops content for Center’s website, poster presentations, press releases, blogs, and marketing materials
Provide oversight to JPB Care Coordinator on project initiatives
Designs tools and measures that align with Center for Care Coordination desires and expectations.
Develop and maintain key relationships with internal and external collaborators
Assess Center capacity and resources and determine the most appropriate course of action with respect to supporting evaluation and technical assistance needs
Effectively problem solve across a wide range of areas, soliciting input and guidance for complex problems
Support the research and evaluation design and implementation on new grants and proposals by serving as a content area expert and assuming responsibilities related to project management, with limited supervision
Research and vet new opportunities for the integration and embedding of (models, measures and products) into the care coordination model
Support project leads and/or consultants in the integration, management and ongoing evaluation of the value and impact of multiple initiatives
Participate in monitoring compliance with best practice and integrity in all research and innovation matters
Provides consultation on best practice in engaging families
Contribute to the Center for Care Coordination’s playbook and strategic plan, by leveraging knowledge and experience to increase support of institution
Plans, coordinates and supports leadership in providing technical assistance via webinar, learning collaborative calls, and site visits.
Provide ongoing support around system, data and evaluation efforts of affiliate states.

SUPERVISORY RESPONSIBILITIES
Support and guidance to Care Coordination team on best practices as well as day-to-day support of dedicated JPB funded Care Coordinator
EDUCATION and/or EXPERIENCE REQUIRED
Bachelor’s degree (e.g., Public Health, Public Policy, Business, Social Services, and organizational leadership-project management) required. 2 years of relevant work experience required.
Masters’ degree (e.g., Public Health, Public Policy, Business, Social Services, and organizational leadership-project management) strongly preferred. Consideration provided for 1 year experience with Masters degree.

•Experience in working in clinical or community setting with children and families
•Excellent written and verbal skills, time management and organizational skills
•Conversational Spanish preferred
