Community Horizons is assisting a family in the Plover area by helping to identify a caregiver/companion for their adult son, who has Autism. The perfect fit would be someone who finds joy in the members' interests and can provide compassionate, attentive care while remaining flexible and adaptable to the member and the family needs.

· Gas Compensation- Mileage will be reimbursed for all outings with the member

· Wage: $17.00 -$17.50 HOURS: Weekday Afternoons Up to 2 - 3 days 3 hours per day.

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.

Pay: From $17.50 per hour
Expected hours: 3 – 9 per week
