Schedule: Full-time. Usual hours are 8:00 AM to 5:00 PM, Monday-Friday. The schedule must be flexible to accommodate various organizational needs and able to work holidays and weekends as needed. This position participates in the "On-Call" rotation.


What you will bring to our team:

The right person for this position will have a participatory leadership style and enjoy problem solving. They will be passionate about ensuring high quality services are provided. They must have the ability to work with minimal supervision and an understanding of Department of Human Services and/or Department of Health rules and regulations.

  • Valid driver’s license and meet qualifiers to be insured by EON, Inc.’s vehicle insurance provider.
  • Must not be disqualified by appropriate criminal background studies.
  • Meet the physical requirements as outlined in this position description.
  • Possess good oral and written communication skills and organizational skills.
  • Able to read and write in English.
  • Able to document information electronically.
  • Current LPN License
  • Basic knowledge of Microsoft Office and Internet Browsers
  • Knowledge and experience in the field of comprehensive homecare, day services, and 245D non-residential services.

The Health Services Supervisor (HSS) is responsible for supervising the provision of quality nursing care for service sites as well as coordinating nursing and training programs.


The right person for this position would be eager to help in the following areas:

  • Assisting with ADL's, medication administration, and medical appointments.
  • Documenting person served health concerns,
  • Attending various meetings, trainings, and consultations
  • Collaborating with other agency programs, and person served family members
