Position Summary:We are seeking a compassionate and skilled PRN Physical Therapy Assistant (PTA) to join our home health team. This role involves working under the supervision of a licensed Physical Therapist to deliver high-quality, personalized therapy services to patients in their homes. The PTA will focus on assisting patients with rehabilitation, mobility, and physical function improvement while collaborating closely with patients, families, and healthcare providers. The ideal candidate will be a strong communicator, able to follow treatment plans accurately, and dedicated to helping patients regain independence and improve their quality of life.
Key Responsibilities:
Direct Therapy Services:
- Implement therapy interventions as directed by the supervising Physical Therapist, including exercises, stretches, and mobility training.
- Assist patients in improving strength, balance, coordination, and endurance.
- Provide hands-on support with transfers, gait training, and the use of assistive devices.
- Conduct home safety checks and suggest modifications to enhance mobility and safety.
- Educate patients and families on home exercise programs and techniques to maintain progress between visits.
Patient Monitoring & Assessment:
- Monitor and document patient performance and progress, noting changes in functional ability.
- Communicate patient status, concerns, and progress updates to the supervising Physical Therapist.
- Recognize and report any changes in the patient’s condition that may require a revision of the treatment plan.
Collaboration with Healthcare Teams:
- Coordinate care with the Physical Therapist, nursing staff, physicians, and other healthcare providers to ensure continuity of care.
- Participate in team discussions to set achievable patient goals and strategies for meeting them.
Education & Training:
- Instruct patients, caregivers, and family members in exercises, body mechanics, and safety techniques.
- Offer guidance on proper use of mobility aids such as walkers, canes, and wheelchairs.
Documentation & Reporting:
- Maintain accurate, timely records of therapy sessions in Alora (EMR system), patient progress, and communication with the healthcare team.
- Ensure compliance with HIPAA and other healthcare documentation standards.
Emergency Response:
- Respond to emergencies during therapy sessions, ensuring patient safety and following established protocols.
Qualifications:
Education:
- Graduate of an accredited Physical Therapist Assistant program.
- Current, valid PTA license in the state of Texas.
Experience:
- No minimum years required; at least one (1) year of experience in home health, rehabilitation, or a clinical setting preferred.
- Experience with geriatric patients, post-surgical rehabilitation, or neurological conditions is highly desirable.
Certifications and Training:
- Current CPR and First Aid certification required.
- Additional certifications in areas such as orthopedics, neuro, or geriatrics preferred.
Skills & Abilities:
- Strong knowledge of physical therapy techniques and principles.
- Ability to follow detailed treatment plans and adapt to patient needs.
- Excellent interpersonal and communication skills.
- Physical ability to assist with patient transfers, mobility training, and exercise demonstration.
- Empathy, patience, and dedication to improving patient quality of life.
Working Conditions:
- Hours: PRN (as needed), flexible scheduling based on patient caseload.
- Environment: In-home care and/or medical facility settings; requires travel to patient locations in Houston and surrounding areas.
- Physical Demands: Frequent standing, walking, bending, lifting, and assisting patients with mobility.
Compensation:
- Competitive hourly rate based on experience and qualifications.
- Benefits may be available depending on employment status and duration of service.
