About Cottonwood:
Cottonwood Community Alternatives, Inc. is a private, non-profit agency established in 1991 to provide residential and day program services to Colorado citizens with intellectual and developmental disabilities (IDD). Cottonwood’s motto is “People Serving People” and that is what we strive to do both for the individuals we support and our providers.
Cottonwood is looking for a host home provider for permanent placement of an individual in services. A Host Home (HHIC) is an individual or family that contracts with Cottonwood and provides services within their own home. The individual that is placed in a Host Home setting is integrated into the family, their activities, and receive personal support and care. Host Home Providers are required to meet the same training and background checks as our employees.
About the Individual:
- This individual is a fun-loving, social, and empathetic fifty-eight-old male with intellectual/developmental disabilities.
- He enjoys going into the community as much as possible (particularly for shopping, attending church, eating out, and going to the library), playing games, making new friends, and having reasons to celebrate.
- He has 4 hours of alone time at home and requires line of sight supervision when in the community.
- He is hard of hearing and wears a hearing aid, but he can verbally communicate his needs and wants clearly.
- He does not present a concern for high behaviors.
- Some of the individual's personal care needs include but are not limited to: medication administration, meal preparation, prompts and assistance with some ADLs, adhering to his diabetes protocol.
- This individual attends day program Monday-Thursday and church on Sundays.
About the Ideal Host Home Provider:
- Someone who can meet this individual's supervision and personal care needs.
- Previous host home experience is preferred.
- Reliable access to a computer and internet and comfortability with online documentation platforms (Therap).
- Reliable vehicle to provide transportation when needed (medical appointments, community outings, etc.)
- Your home must be a clean and safe residence that is able to pass a home inspection.
- Comfortable with hands-on care such as hygiene and medical care/medication administration.
- Familiarity with the IDD population and behavioral support.
- Ability to pass a background check, CAPS (CO Adult Protective Services) check, and driving record check.
- Preferred location: Westminster area
- Pets, children, and another individual in services living the home is OK - this individual typically gets along with everyone.
Overview of Host Home Responsibilities:
- Meeting individual supervision needs
- Providing transportation as needed
- Daily documentation
- Administering medication and medical treatment as needed
- Providing fun and leisure activities as well as community integration
- Assisting with ADLs
- Comply with all state and agency policies and procedures
- Working with individual to help reach and maintain goals
Contract Rate:
- Cottonwood's suggested daily contract rate for this individual's support level is $165.70/day.
