MyCGMCoach helps patients take control of their health by providing continuous glucose monitors (CGMs) and personalized coaching. We believe in meeting people where they are—literally. Our house call program is designed to ensure patients get hands-on support, guidance, and confidence as they begin their CGM journey.
Role Overview
We are seeking a compassionate and skilled Nurse Practitioner (NP) to deliver in-home visits for patients in Leisure World, Silver Spring. You will be the face of MyCGMCoach in the community, responsible for getting patients fully set up with their CGM, answering questions, and coaching them through their first steps. This role combines clinical expertise with patient education and empowerment.
Responsibilities
- Conduct house calls to patients within Leisure World, Silver Spring.
- Provide hands-on assistance with CGM device setup, insertion, and activation.
- Educate patients on device use, best practices, and lifestyle integration.
- Perform basic health assessments to ensure CGM readiness and safety.
- Offer guidance on nutrition, glucose tracking, and interpreting trends.
- Document patient encounters accurately in the electronic record.
- Collaborate with MyCGMCoach care team to ensure continuity and follow-up.
- Deliver an outstanding patient experience that builds trust and confidence.
Qualifications
- Licensed Nurse Practitioner (NP) in Maryland.
- Strong background in primary care, endocrinology, or diabetes management preferred.
- Excellent communication skills; ability to explain complex concepts simply.
- Comfortable with technology and teaching patients how to use health devices.
- Empathetic, professional, and patient-first mindset.
- Reliable transportation for in-home visits within Leisure World.
