Active participate with the on-call rotation (Friday-Sunday)- incentive based on-call. On-call rotation includes every 5th weekend, does not include every other holiday.
The MDS/Care Plan Coordinator is responsible for completing MDS documents and preparing resident plans of care timely and efficiency.
The MDS/Care Plan Coordinator is responsible for conducting quality audits and reporting results to the DON.
This position includes the tasks and duties of the center's skin nurse which includes weekly skin assessments.
Excellent Communication Skills required.
Experience Based Pay.
On average the MDS Coordinator would complete a total of 5 MDS a week (includes a variety of Quarterly, Annual, Significant Changes and Admission)
