Oak Grove Recovery operates Residential Treatment Programs for individuals with Substance Abuse Disorder. We believe that treatment is local, individualized, holistic, and relational. Currently, we have a presence in several states and are rapidly expanding nationwide.
Summary:
The Patient Navigator acts as a member of a multidisciplinary team to assess and coordinate patients? care among multiple service providers during inpatient and outpatient treatment.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assessing patient out of facility biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and developing a strategy to meet these needs during and after treatment episode
- Facilitating multiple care aspects (case coordination, information sharing, aftercare planning, court related issues)
- Maintains a good working relationship with support networks, government resources, community resources, employers, and referral sources.
- Completion of all necessary documentation related to FMLA and Short-Term Disability
- Maintains patient records by reviewing and completing case notes
- Prepares patients? discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting
- Participates in weekly multidisciplinary treatment team as part of ongoing assessment of patient needs and plan for discharge
- Daily involvement in census management including coordinating discharge schedule and communicating to treatment team and administrative team.
