Position Summary
We are seeking a Qualified Mental Health Professional–Community Services (QMHP-CS) to provide Skills Training & Development and Targeted Case Management to children and families. The ideal candidate is flexible, detail-oriented, and passionate about serving the community.
Key Responsibilities
- Provide Skills Training and Targeted Case Management in-home, in-school, and in the community.
- Develop and implement Individual Service Plans (ISPs) in collaboration with the client, guardian, and treatment team.
- Conduct regular check-ins and document client progress in compliance with Texas Medicaid and MCO requirements.
- Coordinate with families, schools, and providers to ensure wraparound support.
- Attend required supervision, team meetings, and training sessions.
Qualifications
- Bachelor’s degree in Psychology, Social Work, Human Services, or related field.
- Must meet Texas QMHP-CS qualification standards.
- Experience with children/adolescents in behavioral health is strongly preferred.
- Strong documentation, organizational, and communication skills.
- Reliable transportation for community-based visits.
