The QMHP will provide in-home, community-based therapeutic interventions to youth at risk of out-of-home placement due to behavioral or emotional disturbances. The QMHP will implement individualized service plans, collaborate with families and service providers, and support clients in achieving emotional stability, safety, and skill development in the least restrictive environment.
Key Responsibilities
- Deliver intensive therapeutic interventions and behavioral supports in clients' homes and community settings.
- Conduct assessments and assist in developing Individualized Service Plans (ISPs).
- Implement and monitor crisis intervention plans and coordinate wraparound services as needed.
- Maintain timely and accurate documentation, including progress notes, ISPs, and monthly reports, per Medicaid and agency standards.
- Collaborate with treatment teams, schools, social services, and other stakeholders to support client progress.
- Participate in regular supervision and training to meet licensing and programmatic requirements.
- Ensure adherence to all DBHDS, DMAS, and Medicaid regulations.
- Provide on-call support as scheduled for crisis intervention and support outside standard work hours.

Pay: $25.00 - $30.00 per hour
