Job Description:
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager for a 13-week contract position in Silver Spring, MD. In this role, you will collaborate with interdisciplinary teams to coordinate and manage comprehensive patient care plans, ensuring the highest quality of care and resource efficiency. This is a day shift position, offering a stable weekday schedule and the potential for extension based on performance and facility needs.
Responsibilities:
- Develop and manage patient-centered care plans based on individual assessments
- Coordinate services across the care continuum to ensure seamless transitions
- Conduct utilization review and ensure documentation supports medical necessity
- Collaborate with physicians, social workers, and other healthcare professionals
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on care options and discharge planning
- Advocate for patient needs while optimizing outcomes and controlling costs
Qualifications:
- Active RN license in the state of Maryland
- Minimum 2 years of case management experience (acute care setting preferred)
- Strong knowledge of healthcare delivery systems, insurance processes, and community resources
- Excellent communication, problem-solving, and organizational skills
- Proficient in EMR systems and case management software
