Some of the duties:
- Oversee and manage hospice patients
- Develop and implement nursing care plans for patients
- Monitor patient progress and make adjustments to treatment plans as necessary
- May provide direct patient care, including administering medications, performing procedures, and monitoring vital signs
- Collaborate with physicians and other healthcare professionals to ensure coordinated and comprehensive patient care
- Conduct regular assessments of staff performance and provide feedback and coaching as needed
- Ensure compliance with all regulatory guidelines and standards of practice
- Maintain accurate and up-to-date patient records
-Determines scope and frequency of services needed based on acuity and patient/family needs
-Assess the ability of the care giver to meet the patient's immediate needs upon admission and through out patient's care
- Consult and educate the patient and family regarding diseases process, self-care techniques, end of life care, nutrition and dietary needs
-Apply concept of infection control and standard precautions in coordinating and performing patient care to prevent contamination and transmission of disease
```Experience```
- Minimum of 2 years of experience as a hospice registered nurse in a home/clinical setting
- Strong knowledge of medical procedures, terminology, and equipment
- Experience in nurse management or supervisory role preferred
- Proficient in case management and coordinating patient care
- Familiarity with medical office operations and electronic health records systems
- Skilled in performing home/clinical assessments, including vital signs monitoring, geriatric care, sterile processing, etc.
- Experience administering injections and palliation for end of life
- Excellent communication and interpersonal skills
-Current state license as a registered nurse (RN) and/or in accordance with the Board of Nursing rules for Nurse Licensure Compact. Current Driver's License
-Graduate from accredited School of Nursing
