Beth Israel Lahey Health Primary Care is creating a top primary care model to improve patient experiences and care quality. They are also starting a diabetes education program. The Diabetes Educator Team Nurse will collaborate with care team members to educate and support diabetes patients, conduct group sessions, and create educational materials.Job Description:
Primary Responsibilities:
Team Nurse Duties:
Coordinate outpatient care and preventive services.
Collaborate with community providers for follow-ups.
Provide telephonic triage, assessment, and education.
Manage INR for Coumadin patients.
Administer vaccines and medications, providing education.
Perform nursing tasks under provider direction, document care.
Monitor abnormal test results and manage referrals.
Oversee clinical supplies and ensure proper specimen handling.
Educate and train unlicensed staff, maintain compliance.
Ensure patient health information is used correctly and promote patient safety.
Diabetes Education Duties:
Assess diabetes patients' clinical and psychosocial needs.
Identify educational needs for diabetes self-management.
Develop care plans based on expertise and patient lifestyles.
Empower patients for optimal health control.
Educate on medication, blood glucose monitoring, nutrition, and prevention.
Monitor therapy response and adjust care plans.
Support preventive health behaviors to reduce complications.
Conduct quality improvement studies.
Recognize the need for continuous diabetes education.
Provide educational sessions and resources for care teams.
Required Qualifications:
1. Graduate from an accredited Nursing Program required. Bachelor's degree preferred.
2. 1-3 years related work experience required.
3. Strong interpersonal skills. Strong patient communication and patient education skills.
4. Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.
II. Licensure, Certification, Registration
1. License Registered Nurse required, or Registration Licensed Practical Nurse required, and Certificate 1 Basic Life Support required.
2. Certified Diabetic Care Educator and Specialist (CDCES) required.
3. CPR Certification
