Weekend Community Buddy / Caregiver

The ideal candidate will be personable, creative, energetic, and maintain a positive attitude. This position is scheduled for weekends, either every weekend or every other weekend.
This position is within the self-direction program. Under self-directed services you work directly for the individual, which offers more work life balance and provides a great one on one teaching opportunity. This position is hired directly by the member and not through an agency.
Schedule:
Weekends (could be Friday, Saturday or Sunday)
Approximately 9 hours split between 2 days
Work together with the family to create a schedule that suits both you and the family

Pay: $15.00/Hour
Your Tasks Will Include but are not limited to:
- Providing transportation to community activities
- Supervision within the community
-Assisting with medications as prescribed
-Gentle reminders of personal care
-Meals according to individualized diet
Some interests include:
- Bowling, movies, Shopping, Going for bike rides and Bike riding
You Might Be the Perfect Fit If:
- You’re active and love being on the go
- You’re not easily intimidated
- Knowledge of seizures is a plus or willing to learn
- You’re familiar with Autism
- You have a valid driver’s license and a reliable vehicle
- You’re comfortable around pets and have no allergies
- You have a friendly nature and enjoy being around others
Who is Community Horizons
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
