SUMMARY:
Provides well woman care to specified patient populations in ambulatory setting including ordering and evaluating diagnostic tests and management of minor complications. The Nurse Practitioner utilizes theories of psychosocial, developmental, educational and nursing sciences to facilitate the delivery of this health care. The Nurse Practitioner works inter-dependently with and independently of the multi-disciplinary team. Major functional areas will include: Practitioner, consultant, and educator.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Based on protocols, standing orders, or in collaboration with a physician, arranges for necessary laboratory, radiology and/or other diagnostic tests to complete delivery of primary health care
- Works to establish and to promote a multi-disciplinary approach to patient care
- Refers deviations from normal to the appropriate health care professional (e.g. social worker, nutritionist)
- Works to further establish advanced clinical expertise and skills within area of clinical practice
- Serves as a role model by demonstrating expertise and comprehensive knowledge in the delivery of health care to patients and their families
- Assists patients to assume responsibility for the prevention of illness, the promotion, maintenance and restoration of health by being culturally sensitive to their experience
- Advocates the primary nurse concept of patient care in a clinic setting and facilitates implementation of that role
- Assesses the learning needs of the patient population of Alivio Medical Center, in collaboration with all members of the health care team
- Plans, provides and coordinates patient and staff education, with other members of the health care team
- Assist with the evaluation of education activities to determine their impact on the quality of care
- Recognizes self-development needs and seeks out resources or experiences which promote professional growth
- Perform other related duties as assigned
This position requires compliance with all of Alivio’s written standards, including its

QUALIFICATIONS:
Education:
- Master or Bachelor Degree in Nursing and graduate of an accredited Masters Bachelor’s Degree Program
- Board certified as a WHNP.
Licensure:
- Fully licensed in the State of Illinois
- Active DEA License
Experience:
- Minimum of two years experience. Previous experience in an FQHC setting, preferred, but not required.

- Bilingual English/Spanish, required
- Demonstrated ability to communicate and interact with patients in a culturally sensitive manner.
