HART Hub Support Worker
2 – 1.0 FTE Contract Positions (6 - Month Contract)

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking two (2) full-time contract Support Workers at the Homelessness and Addictions Treatment and Recovery Hub (HART Hub) to join HART Works, a low-barrier transitional work program within the HART Hub model of care. Reporting to the Program Coordinator, the Support Worker plays a key role in the daily delivery of HART Works, supporting and engaging participants experiencing homelessness, housing instability, substance use challenges, mental health concerns, and other complex barriers.
The position involves direct participant support, community-based work activities, program facilitation, and administrative responsibilities. Duties include regular outdoor work in all seasons and participation in physically demanding activities such as snow shovelling, property maintenance, community clean-up projects, and hands-on tasks. Other related duties may be assigned as required in support of HART Works program.
The successful candidate will demonstrate reliability, adaptability, and a non-judgmental approach while supporting participants in achieving stability, recovery, community inclusion, and future opportunities.
The position has a competitive salary, and the opportunity to participate in the Health Care of Ontario Pension Plan (HOOPP).
The pay range for this position is $24.756 to $30.945 per hour. Compensation will commensurate with qualifications and relevant experience.

QUALIFICATIONS
Knowledge of community services, community resources, and systems navigation.
Strong advocacy, communication, and interpersonal skills.
Experience with overdose prevention and response, including Naloxone administration.
Knowledge of substance use, harm reduction principles, homelessness, and related social issues.
Demonstrated ability to de-escalate conflict, manage challenging situations, and maintain professional boundaries.
Awareness of and sensitivity to the needs of individuals facing barriers to healthcare and social services, including people who are underhoused, living with mental health challenges, experiencing substance use, newcomers, uninsured individuals, and members of the 2SLGBTQIA+ community.
Proficiency with computers and common software applications, including documentation and data entry.
Ability to work independently and collaboratively within a multidisciplinary team.
Valid Ontario Class G driver's license.
Lived experience with homelessness, housing instability, substance use, or recovery is considered a strong asset.
Experience working within community, social service, harm reduction, mental health, addictions, or outreach settings is considered an asset.
