The Care Coordinator is responsible for managing communication between patients, care providers and other healthcare institutions. The Care Coordinator is highly skilled in providing an excellent customer experience and answering questions, can articulate themselves in written and verbal outreach, and is able to respond to changing circumstances and needs. The Care Coordinator will represent the company professionally and positively, always exercising utmost discretion, diplomacy and tact in customer interactions.

The ideal Amazonian will demonstrate the ability to work independently with the utmost of professionalism, integrity, and dependability. An Amazonian has a bias for action and is strongly self-motivated not only to take great care of our customers, but also to proactively identify systemic issues that need to be resolved to prevent future customer concerns (Amazonians don’t pass the buck when it comes to customer issues). Finally, an Amazonian is supremely internet savvy and has high technical aptitude when it comes to online tools and research and has a passion for continuous learning.

Key job responsibilities
As a Care Coordinator, you will be responsible for the following:

  • Analyze history of account and prepares documentation as needed to effectively communicate
  • Process tasks and prepares account information as needed to communicate with outside partners
  • Manage patient accounts with contact information, insurance and financial information in a confidential manner
  • Investigate and problem solve to identify missing information or remove barriers preventing customers from getting what they need
  • Establish and maintain relationships with identified service providers.
  • Maintain ongoing tracking and organized documentation system to support workflows and audits

  • Bachelor’s degree in applied field
  • 3+ years as a Care Coordinator or Referral Coordinator or Insurance Coordinator
  • Current and well versed in CPT and ICD codes, proficient with medical terminology
  • Experience with electronic medical record systems.
  • Excellent interpersonal skills, with the ability to communicate and present complex issues correctly and clearly to both internal and external customers.
  • Ability to effectively manage time, and individually prioritize multiple tasks of competing priority.
  • Ability to adapt quickly to new processes and tools.
  • Proven ability to protect confidential information, such as PHI
  • Experience working with multiple stakeholders to lead process improvement initiatives with the end customer in mind.
  • Demonstrable skills using word processing and spreadsheets.
