Position Summary & Responsibilities

Position Summary:

The person in this position is responsible for diagnostic coding which is essential for financial/reimbursement purposes. Assigns DSM and ICD codes following official established coding guidelines for all admissions, established diagnoses, and discharges This individual assigns and sequences appropriate Complication/Co-Morbidity (CC) codes for optimal DRG assignments. This person provides education and training to Medical Staff and monitors documentation content and timeframes for documentation compliance according to The Joint Commission and Centers for Medicare and Medicaid Services standards and regulations.

Job Responsibilities may include but are not limited to the following:

CODING

  • Assigns all DSM and ICD codes following official established coding guidelines for all admissions, established diagnoses, and discharges;
  • Assigns all ICD codes following official established coding guidelines for all EKGs, Imaging Studies, Consultations and on-sight procedures;
  • Assigns appropriate CPT codes for physician services provided;
  • Provides codes to unit CIM personnel for DSM diagnoses, as needed;
  • Enters/verifies all admission, established, and discharge diagnostic codes into the record;
  • Assigns and sequences appropriate Complication/Co-Morbidity (CC) codes for optimal DRG assignments;
  • Completes Medicare Billing Form sent by Reimbursement, utilizing discharge diagnoses and CC codes, returning such to Reimbursement for further processing/billing;
  • Records diagnostic coding on billing forms for all lab procedures utilizing online documentation;
  • Adds whole blood glucose codes into the whole blood glucose billing program, utilizing online documentation, returning same to Reimbursement for further disposition;
  • Queries providers when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or indicate unclear coding purposes;
  • Provides education and training (as requested) to providers for appropriate documentation;
  • Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and/or Association of Registered Health Care Professions (ARHCP);
  • Provides data to Chief Medical Officer and Compliance Officer with regard to documentation compliance for billing;
  • Codes and records status of multi-drug-resistant organism and methicillin-resistant Staphylococcus aureus.

MONITORING

  • Reviews online Seclusion/Restraint documentation for required elements and CMS and TJC regulations;
  • Monitors timely/thorough completion of Admission Intake Assessments, physical examinations, admission orders;
  • Monitors suicide risk (high) assessment and suicidal precautions ordered;
  • Prepares reports on a regular basis for administrative review;
  • Monitors CITP completion for billing;
  • Monitors Medical Staff Orders for missing signatures;
  • Monitors and directs designated documentation processes real-time;
  • Monitors and provides education for documentation for medical necessity;
  • Ensures medical necessity of ancillary testing prior to billing;
  • Provides other chart review/monitoring/auditing, as needed.

OTHER DUTIES

  • Provides coverage for additional coding tasks;
  • Covers entry of daily movement into MRM and Plexus, as needed;
  • Reports out on daily census when providing coverage;
  • Assists with or performs special projects as assigned;
  • Provides coverage for census;
  • Provides additional coverage for other areas of department as needed;
  • Assists with or performs special projects/other duties as assigned;
  • Initiates discharge summaries for SPTP residents and provide back-up for PSP discharge summaries;
  • Conducts monitoring functions as needed for performance improvement;
  • Responds to clinical staff requests.


Qualifications

Minimum Qualifications:

  • At least 1-2 years’ experience in coding.
  • Educational background in medical terminology or Heath Information Management.
  • Educational background in coding (i.e., ICD, CPT, etc.).

Preferred Qualifications:

  • Certification in one of the following: Registered Health Information Administrator (RHIA); Registered Health Information Technician (RHIT); Certified Coding Associate (CCA); Certified Coding Specialist (CCS); Certified Coding Specialist-Physician based (CCS-P); Certified by ARHCP; Registered Medical Coder-Associate (RMC-A); Registered Medical Coder (RMC); Registered Medical Manager (RMM); Certification by AAPC; Certified Procedural Coder (CPC); or Health Coding Specialist.
  • Graduate from an AHIMA accredited Health Information Management program and/or currently enrolled in medical coding classes (i.e., ICD, CPT, etc.).

Post-Offer, Pre-employment Requirements:

  • Ability to pass a KBI background and pre-employment drug test.
