About Montgomery Place: The premiere senior living community overlooking Lake Michigan and the Museum of Science and Industry, Montgomery Place serves as the cultural center of East Hyde Park. Offering Independent Living, Assisted Living, as well as a 40-bed Skilled Nursing Unit providing Long-term Care and Short-term, sub-acute rehab services.
Position Summary: Responsible for building and developing the Skilled Nursing and Assisted Living census and quality mix by providing prospective residents and/or responsible parties with appropriate information and assistance regarding admission to Skilled Nursing and Assisted Living.
Pay range: $50k - 60k

Essential Duties and Responsibilities
Overall responsibilities
- Handles walk in or telephone inquiries and invites referral sources to Skilled Nursing and Assisted Living
- Follows up on inquiries by conducting tours of the center, completing inquiry forms for admission paperwork, and providing all inquiries with brochures about nursing home, Medicare facts, and room rates
- Reviews referrals, assisting with patients/guardians/advocates interviews to secure all necessary information and paperwork as needed for admitting to the facility
- Contacts Billing Specialist to confirm insurance eligibility
- Submission of pre-admission insurance authorization and updates until resident discharge
- Completes a criminal background check for all new patients and residents prior to admission using the Identified Offenders data base
- Gathers nursing information from outside social worker and floor nurse at hospital
- Utilizes sales skills and persuasive techniques to obtain desired admissions to Skilled Nursing and Assisted Living
- Maintains and distributes daily census
- Uses formal waiting list policy and keeps future reservations for residents
- Reviews list of unoccupied beds and makes pre-admission reservations according to type of case and accommodation required
- Maintains direct knowledge of served market areas, resident origin, and major referral source
- Maintain records of activities (REPs)
- Follows Montgomery Place procedures to work with Administrator and Director of Nursing to follow prospects and referrals through the admission process
- Uses formal application to screen residents financially
- Visits prospective residents in the hospital or other facilities and make assessment regarding his/her suitability for admission in the nursing center
- Contacts Social Services Department to verify eligibility
- Contacts insurance companies to determine or verify coverage
- Assists Social Services with meeting families, meet with all residents and/or families during admission to sign paperwork and record key information
- Explains differences in rates and charges to residents desiring changes of accommodations, and arrange for the change
- Explains services and nursing home policy regarding payment of bills
- Inspects resident’s room prior to admission; notify particular wing to expect resident and arrange for escort to room
- Consults with Administrative, Director of Nursing, Rehab Director, and other departments as needed regarding a referral eligibility
- Works with organizations in bundle payment programs and those in preferred provider networks (i.e. hospital and home health); increase participation in preferred provider networks
