Summary
We are seeking passionate and dedicated Behavioral Health Case Managers I & II to join our growing team. In this role, you'll support clients in navigating mental health and social services, ensuring they have the tools and resources needed to move forward in their recovery journey.
Full-Time Case Managers I & II will receive 60% of their billing along with benefits.
Responsibilities
- Provide case management and advocacy for clients with behavioral health needs
- Develop and monitor individualized care plans
- Coordinate referrals and community resources (housing, employment, transportation, etc.)
- Maintain accurate and timely documentation
- Participate in team meetings, supervision, and ongoing training
- Communicate effectively with clients, providers, and support systems
Requirements
Case Manager I:
- High school diploma or GED required
- Bachelor’s degree in a human services field preferred
- Willingness to obtain CM I certification if not already held
- Strong interpersonal and organizational skills
Case Manager II:
- Bachelor’s degree in psychology, social work, or a related field (required)
- Certification as a Case Manager II through ODMHSAS (preferred or eligible)
- 1+ year of case management or behavioral health experience preferred
- Ability to work independently and as part of a collaborative care team
- Bachelor’s degree in a human services field preferred
- Willingness to obtain CM I certification if not already held
- Strong interpersonal and organizational skills
