Job Summary:
We are seeking a dedicated On-Call Behavioral Health Case Manager to provide intake assessments, treatment planning, discharge coordination, and client advocacy outside of regular business hours. This role is critical for ensuring continuous client care and smooth transitions.
Responsibilities:
- Conduct initial client assessments within 4 hours of intake
- Draft and coordinate treatment plans in collaboration with Behavioral Health Professionals (BHP)
- Initiate and update discharge planning, ensuring client needs are met post-discharge
- Complete discharge summaries and documentation timely and accurately
- Communicate effectively with clinical staff, clients, families, and community providers
- Follow all AHCCCS and ADHS compliance requirements
- Participate in on-call rotation schedule
Qualifications:
- High school diploma or GED required; Associate’s or Bachelor’s degree preferred
- Minimum 1 year of experience in behavioral health, case management, or related field
- Knowledge of mental health, substance use disorders, and community resources
- Strong documentation and communication skills
- Ability to work independently and handle crisis situations
- Valid fingerprint clearance card or ability to obtain
- CPR/First Aid certification preferred or willing to obtain

Pay: $18.00 - $22.00 per hour
