The Care Coordination Specialist at Neuro Performance and Injury Center plays a critical role in ensuring that all patients, especially those who are uninsured and underinsured, receive timely and effective healthcare. This role involves managing and coordinating all aspects of patient care, from initial assessment to the final stages of treatment. The specialist will act as a liaison between patients, healthcare providers, and insurance companies to facilitate optimal care and support. This position communicates consistently and effectively with various partners and community providers. Care Coordinator is a member of the home office location who will follow patient care from intake to scheduling for all locations.
Job Duties
- Work closely with healthcare providers to develop and implement personalized care plans
- Liaise with insurance companies to optimize coverage and minimize patient costs.
- Schedule appointments and follow-ups, manage patient files, and maintain communication with all parties involved in patient care.
Requirements
- Bachelor’s degree in healthcare, related field preferred
- Minimum of 1-2 years’ experience in Care Coordination in Healthcare private setting
- Experience in care coordination, case management, or a similar role, particularly with uninsured or underinsured populations.
- Sales Force Experience preferred.
- In-depth knowledge of healthcare systems and insurance processes.
- Strong communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Proficiency in healthcare IT systems, including electronic health records (EHR).
- Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
- Commitment to providing high-quality care to underserved populations.
- Provide support and resources to patients, helping them navigate the healthcare system effectively.
The Neuro Performance Injury Center is located in Lewisville, TX and is a practice specializing in neurology and neuropsychology.
We are looking to add a highly motivated and reliable member to our team to manage a team of patients throughout treatment. Additional Job Duties:
- Manage timelines and deadlines of each of your assigned patients including receiving patient consents, letter of protection (LOP), and available medical records
- Effectively manage a case load of 100-250 cases
- Deliver patient case management service while maintaining a strong focus on patient medical treatment to ensure patients are compliant with plan prescribed by treating doctors, contacting patients every two weeks to discuss care coordination updates, medical treatment status, and to answer questions
- Take ownership over patient files for review and communication with referring accounts
- Work consistently and proactively to address patient care issues; draft updates to referring and collaborating providers, attorneys and internal contacts regarding patient care
- Acquire, review and distribute medical and billing records throughout the treatment phase
- Accurately maintain case management data in the case management system
- Work with the Scheduling and Intake teams to create a cooperative team environment ensuring all policies and procedures are followed through to completion
- Schedule/re-schedule each patient quickly and efficiently as required
- Identify issues and solve problems quickly
- Communicate with patients, medical assistants and providers on a regular basis
- Communicate effectively with our internal billing and records specialists to ensure the correct and complete bills and medical records are obtained timely
- Enter detailed notes in EHR and keep cases updated daily
- Coordinate transportation for patients as required
- Answer all inbound communication and redirect as required
